Canadian Patient Safety Institute 2018-2019 Annual Report


There is widely shared agreement that the Canadian healthcare system needs a sharper patient safety focus. The rate of preventable harm in all care settings is alarming, yet poorly understood, leading to complacency and acceptance of patient safety risks. We should expect more, and this is the imperative of the Canadian Patient Safety Institute as the “guardian of patient safety in Canada.“

2018-2019 brought about a change in the strategic direction of the Canadian Patient Safety Institute. Our aim is to inspire and advance a culture committed to sustained improvement for safer healthcare. In this first year of our new five-year business plan, we laid the groundwork to demonstrate what works and strengthen commitment for end-to-end patient safety improvements and we are using those strategic elements to make care safer. We invite you to read this Annual Report to learn more about our priorities and progress.

2018-2019: BY THE NUMBERS

A 2018 survey commissioned to determine Canadians’ awareness of the patient safety crisis in our healthcare system produced two notable conclusions: Canadians were largely unaware of the crisis, yet once they were informed, this became one of their top healthcare priorities. The Canadian Patient Safety Institute and Patients for Patient Safety Canada (a patient-led program of the Canadian Patient Safety Institute) are using these results to initiate conversations about the patient safety crisis and aim to strengthen commitment to the reduction of preventable patient harm across Canada.

Patient Safety Right Now

The Canadian Patient Safety Institute’s Patient Safety Right Now strategy was built to demonstrate that change is both urgent and possible. In this new strategy, the Canadian Patient Safety Institute works strategically by demonstrating what works to sustain patient safety improvements within the healthcare system. We also draw the system towards greater safety by raising the profile of patient safety, securing greater commitment from everyone.

Implementing and Evaluating Effective Safety Interventions

Line of business: Safety Improvement Projects

The Canadian Patient Safety Institute is applying quality and safety improvement methods, knowledge translation implementation science, and evaluation methods to improve system capability and translate the evidence of what works into standard practices for practitioners/providers, leaders, and the public at all levels of the health system. Four new Safety Improvement Projects have been launched on priority topics that are aligned with associated patient harm occurring across the country.

Line of business: Making Patient Safety a Priority

The Canadian Patient Safety Institute is increasing the profile of patient safety among the public and patients, healthcare providers, leaders and governments. Social marketing campaigns are working to create positive behaviour changes that help keep people safer in our healthcare systems. The patient voice is at the forefront of campaigns and our media strategy to make patient safety a higher priority across the healthcare system. The Government Relations and Public Engagement strategies are platforms for letting the patient voice be heard.

The Canadian Patient Safety Institute strengthens commitment by working with patients, governments, and health professions regulators, standards setting organizations, professional groups and other partners to impact levers such as policy, regulation and accreditation that enforce or ensure accountability for safer care. Under the guidance of the Policy, Legal and Regulatory Affairs Advisory Committee, the Canadian Patient Safety Institute Policy Framework has been developed to outline our approach to informing and influencing policy across Canada.

The Canadian Patient Safety Institute is strengthening strategic alliances and networks with patients, governments, industry and other partners who are committed to making care safer. Our partnership with patients through Patients for Patient Safety Canada and other groups help to advance patient safety improvements at all system levels. Through a strategic partnership with the HSO, the Canadian Patient Safety Institute has created a draft Canadian Patient Safety and Quality Framework.
